PSIsafe Troubleshooting: Issue 07.02: When a Form is opened, error shows in Left Corner Load failed

Problem: 

When a Form is opened, the following error shows up in the top left corner: “Load failed.”

Solution:

The document as an invalid character “<” or="or" “="“">” typed in one the entry boxes. Open document in a text editor. Scroll to the end of the document where you will see the user typed values. Look for the invalid entries and replace with valid alphanumeric values.
